Family Case Management provides coordination of health and social services for eligible families with a pregnant woman or infant. Our goals are to reduce infant mortality by ensuring early and consistent prenatal care for women throughout their pregnancy. FCM facilitates necessary early periodic screening diagnostic testing, and required immunizations PDF Document for children. Our families get help in finding an ongoing source of medical care and financial assistance. They are connected to nutrition services, health education, transportation, mental health services, substance abuse treatment and other services they may need. FCM participates in community initiatives such as county wide developmental screenings coordinated by the Regional Office of Education. FCM promotes injury prevention and child passenger safety through participation with Quad City Safe Kids Coalition ExternalLink.gif. Women and children come into our services through the Department of Human Services, requests for pregnancy tests, and referrals from physicians, agencies or individuals. Rock Island County has been successful in lowering our infant mortality rate. The numbers of infants and children appropriately immunized continue to increase and are well above the state average.

Illinois' universal child health insurance plan, called All Kids can be initiated at the Health Department. Our staff provides assistance in filling out the applications.

Family Case Management formerly known as Healthy Moms/Healthy Kids also coordinates an incentive program for pregnant women who live and receive prenatal care in Rock Island County. This program is called the NEST. It is funded and staffed by the Junior Board of Rock Island. Clients who participate in the program redeem their points for baby items at the Rock Island K-Mart.
